Access to medical records is available to patients over the age of 18 or a legal guardian. Records are available in two ways:

Copies of your complete health record
Copies of medical records may be released upon receipt of written authorization of the patient or guardian. To request your records, please complete the form below, then contact the Health Information Management office at the facility from which you would like to receive your medical records for additional information on how to submit. If you've been a patient in one of our hospitals or Centura Health Physician Group physician practices, you may download a copy of the medical records release form (also available in Spanish), complete it and either bring it in or fax it to the entity’s Health Information Management department to request your medical record.
